Tretinoin
Can you use tretinoin and minoxidil together?
The combination has one small human trial, a plausible absorption mechanism, and not much else. Here is what that does and doesn't mean.

The one human trial found no clear advantage
The bottom line: adding tretinoin did not beat standard minoxidil in the only randomized trial we found. The study enrolled 31 men aged 28 to 45 with Hamilton-Norwood type III to V androgenetic alopecia, and compared 5% minoxidil twice daily with a combined solution of 5% minoxidil and 0.01% tretinoin applied once nightly (Shin et al. 2007 trial abstract).
Hair counts from macrophotographs and global ratings from patients and the investigator showed no statistically significant difference between groups. Irritation was similar too: pruritus or local irritation occurred in 4 of 14 men on minoxidil alone and 5 of 15 on the combination.
Why people think it works: absorption, not hair
The bottom line: the strongest human data are about absorption, not hair growth. In a randomized crossover study of 19 healthy men, subjects applied 2% aqueous minoxidil twice daily for 20 days, alone or with once-daily 0.05% tretinoin cream or vehicle. The authors concluded that tretinoin increased minoxidil absorption by making the stratum corneum more permeable (1990 absorption study).
A review reports roughly a threefold rise in systemic absorption when tretinoin goes on before minoxidil (review). That matters for safety, since more of the drug reaching the bloodstream is not automatically a good thing. The study also used healthy men rather than people with hair loss, so it cannot tell you whether any of this changes regrowth.
Non-responders and the weak signals
The bottom line: the most-discussed use is adding tretinoin for people who don't respond to minoxidil, and the support for that is secondhand or preclinical. A review says smaller investigations are consistent with some non-responders becoming responders when tretinoin is added (review).
- A 2025 case series reportedly used 0.1% tretinoin gel followed by 5% minoxidil over 90 days in non-responders. We could only see it through a secondary review (PMC review with trial table).
- A 2019 prospective trial of 56 patients reportedly found about half of non-responders improved. We could only see that in a secondary table, so we haven't verified it.
- In male mice, 5% minoxidil alone gave a larger regrowth ratio than minoxidil with 0.1% tretinoin, 39.53 versus 15.70. The authors still called both effective, so this does not show that tretinoin hurts (mouse study).
- In cultured human hair follicles, adding 1 nM all-trans retinoic acid to minoxidil increased growth by 4.2 ± 0.7 mm compared with minoxidil alone (in vitro study). Lab results like this are a starting point, not proof of synergy in people.
Safety: irritation is the everyday problem
The bottom line: irritation is the main everyday concern, and it's mostly what you'd expect from two topical drugs on the scalp. Minoxidil solutions containing ethanol often cause scalp irritation, dryness, itching, and inflammation (Cureus 2023 study). Tretinoin adds its own irritation risk, and online complaints about tolerability are anecdotal (review).
There is also a rare, serious-sounding case. A 1989 report describes an explosive scalp pyogenic granuloma after combined minoxidil and retinoic acid therapy (Baran case report). One case cannot tell you how often this happens, so treat it as a reason to get a scalp checked if a lesion appears, not as a likely outcome.
What to do with this
The bottom line: if you're considering the combination, do it with a dermatologist and expect no guaranteed gain. There is no standard regimen. Studies have used tretinoin at 0.01%, 0.05%, and 0.1%, with different vehicles and timing, and a US patent covers retinoid and minoxidil combinations (US patent 5,183,817). A patent shows what was claimed, not that it works.
- Don't start a strong tretinoin on an already irritated scalp.
- If you're on minoxidil alone and not seeing results, ask about your options before adding a second drug.
- Stop and call your clinician about persistent burning, blistering, or any new scalp lesion.

Frequently asked questions
Can you use tretinoin and minoxidil at the same time?
You can, but there is no standard regimen. The main trial used a once-nightly combined solution, and other studies vary strength and timing. Ask a dermatologist first, especially if your scalp is already irritated.
Does tretinoin make minoxidil work better?
That isn't proven. One small trial found the combination about equivalent to minoxidil alone, not better. The main proposed effect is higher absorption, which is a mechanism, not evidence of more hair growth.
Should I add tretinoin if minoxidil isn't working?
Possibly, under medical supervision. A recent review says dual therapy may be considered mainly for non-responders, but the supporting data are small, secondhand, or preclinical, so expectations should stay modest.
